This policy covers therapy, specialties and coaching offered by CBQUALITY Wellness & Lifestyle.
Scope of Services
Therapy (Florida residents only): Provided via secure telehealth platform (Zoom). Therapy is available only to individuals who are physically located in Florida at the time of service and meet eligibility requirements.
Coaching (worldwide): Professional coaching and mentorship services are available internationally. Coaching is not psychotherapy, does not include diagnosis or treatment of mental disorders, and is not a substitute for therapy or medical care.
We will clarify at intake whether you are engaging in therapy (Florida) or coaching (global). You may not be enrolled in both for the same concern.
Booking, Paperwork & Payment

Booking: Sessions are booked through the website or through direct communication with Claire or her team.

Paperwork: Required consent and intake forms will be sent after booking and must be completed prior to the first session.

Payment: Sessions are prepaid at the time of booking to confirm your appointment, unless otherwise agreed upon in writing with Claire directly.
Fees & Good Faith Estimate (No Surprises Act – U.S.)
If you are not using insurance (self-pay), you have the right to receive a Good Faith Estimate (GFE) of expected charges for non-emergency services. You may request a GFE before scheduling and/or will receive one in writing at least one (1) business day before your scheduled service. Keep a copy for your records. If you receive a one-time bill that is $400 USD or more above your GFE, you may dispute the bill.

Insurance & Superbills
I do not bill insurance, nor do I engage with insurance on your behalf.
Upon request, I can provide an itemized receipt (superbill) that you may submit to your insurer for possible out-of-network reimbursement. Reimbursement is not guaranteed; you are responsible for confirming your benefits and for all charges.
Cancellation, Rescheduling & No-Shows

Please provide at least 24 hours’ notice to cancel or reschedule.

Cancellations with less than 24 hours’ notice and no-shows are charged the full session fee on the day of the missed appointment.

Because appointments are prepaid, missed or late-cancelled sessions are not refundable.
Telehealth Requirements (Therapy)

You must be physically in Florida at the time of a therapy session and confirm your location at the start of each visit.

Use a private, quiet space with reliable internet.

By using telehealth, you acknowledge potential risks (e.g., rare interruptions, security limits). I use reasonable safeguards to protect your information.
Confidentiality & Its Limits (Therapy)
Your information is confidential except as permitted/required by law, including but not limited to:

Risk of serious harm to self or others,

Suspected abuse/neglect of a child, elder, or vulnerable adult,

Court orders or other legal requirements,

As otherwise allowed by state/federal law.
We’ll review consent and notice of privacy practices (as applicable) during intake.
Coaching is not HIPAA-regulated therapy and does not include clinical confidentiality protections. Nonetheless, I respect your privacy and keep coaching information private to the extent permitted by law and contract.
Communication

Do not use social media, DMs, or comments for clinical/administrative needs.

Email is suitable for scheduling/logistics only and may not be secure for sensitive information.

For emergencies or immediate safety concerns, call local emergency services or a crisis hotline. I do not provide crisis services.
Emergencies & Crisis
If you are in crisis or considering harm, immediately call 911 (U.S.), go to the nearest emergency room, or contact:

I do not monitor messages in real time and cannot provide urgent or crisis care.
Records & Releases (Therapy)
You may request access to your records as allowed by law. If you would like me to coordinate care with another provider, you will need to sign a written release of information.
Minors
Services for minors are provided only with the consent of a parent/legal guardian and in accordance with applicable laws.
Fees, Scheduling, & Policy Updates
Current fees are shown at booking. Policies may be updated periodically; the version posted on the website is the governing version.
